Abnormal (treatment of schizophrenia) Flashcards
Schizophrenia
Physiological then it makes sense treatment should be physiological or if the cause is psychological treatment should be to. However from the disthese stress model evidence has been implicated that both factors are the cause so it makes sense that both treatments are used.
Chemotherapy
This is the use of antipsychotic drugs which are effective in treating schizophrenia.
Cholropromazine
Is a conventional antipsychotic drug it is used to combat positive symptoms e.g. Hallucinations. As stated by the dopamine hypothesis the positive symptoms are caused by an overactive dopamine system. So they reduced the dopamine levels by blocking dopamine receptors, resulting in reduced synaptic activity.
A typical clazapine
They are also beneficial on the negative symptoms, it has been suggested that they act on the DZ receptors and only occupy receptors temporarily and dissociate to allow normal uptake. This is why atypical drugs are found to have lower side effects than conventional.
Advantages of chemotherapy
JESTLE ET AL
A relatively inexpensive when compared to psychological therapies this is because more people can be treated because costs are kept low. Maintenance drug treatment prevents schizophrenia relapse so it’s effective, but a side effect is tardive dyskinsia and in 75% of cases irreversible. Jes found the TD rate after nine months of treatment 30% on conventional and 5% on atypical. Showing the benefits.
Davis ét al
Analysed 29 studies found relapse occurred in in 55% of placebo and significantly less 19% with drug. However ROSS AND READ, points out that results can be misleading as it shows 45% given the placebo didn’t have a reaplase which is a significant. amount
labelling theory
They also argue being prescribed medication could reinforce the view there is something wrong, preventing individual for looking and dealing with possible stresses. Could lead to the self fuillfing prophecy and labelling theory. They feel labelled as schizophrenia to the carrier symptoms.
Drug trials have found
A typical drugs are only moderately superior to conventional suggesting they may not be the answer.
CBT
The assumption is that people have distorted belief that influence behaviour in a maladaptive way. CBT is used to help patients identified and correct these faulty interpretations. During CBT therapist lets the patient developer and alternative to the maladaptive beliefs by looking at coping strategies.
where thoughts originated
It encourages to evaluate decisions and consider ways in which they might test the validity of the faulty beliefs. It helps to trace back the origins of their symptoms to get an idea of how they developed where it originated from is crucial. CBT cannot eliminate symptoms but makes patients better able to cope without maladaptive behaviours.
Advantages of CBT
People who had CBT experience fewer hallucinations and recover to a great extent those who just received medication.
Dury et al
Found a reduction in positive symptoms under 50% reduction time in recovery with a combination.
Combination effectiveness KULPERS
Confirmed effectiveness of CPT have a lower dropout in greater satisfaction when using both treatments. However is difficult to assess CBT independent drugs.
KINGDON ET EL
Found not everyone has benefit from CBT because there’re not suitable for CBT because therapist believes they would not fully engaged.
