Abnormal shape of head Flashcards
1
Q
Brachycephaly definition and causes
A
Definition - Decrease AP diameter of head and increased the transverse one
- Downs
- Hypotonic infants due to prolonged immobility
- Bilateral fusion of coronal sutures, associated with mental retardation, cerebral anomalies, cleft palate, Syn/ polydactyly
2
Q
Plagiocephaly definition and causes
A
Asymmetric suture fusion
- Hypotonia or MD due to prolonged immobility/ Preterm infants
- Infantile idiopathic scoliosis
- Premature fusion of coronal suture on one side
3
Q
Scaphocephaly definition and causes
A
Increased AP diameter
- Fusion of sagittal sutures
- Preterm infants
- Hurler’s Syndrome
- Marfan’s Syndrome
4
Q
Trigonocephaly definition and causes
A
Wedge-shape skull with pointing anteriorly
- Fusion of metopic sutures
- Maternal Barbiturates in early pregnancy
- Associated with Hypoplasia of forebrain and Hypotelorism
5
Q
Acrocephaly definition and causes
A
Due to premature of sagittal, Coronal and Lambdoid sutures so that skull points upwards- increased height
- Acrocephalo-syndactyly - Apart’s; Pfeiffer Syndrome
- Acrocephalo-polysyndactyly - Carpenter; Noack Syn
- Craniofacial dysostosis - Crouzon Syn
6
Q
Steps in Clinical examination of Abnormal shape of skull
A
- Note the abnormal shape of skull
- Examination of head as usual - Comment if with evidence of raised ICP
a. Note
Size; Shape; Dilated veins; Sun-set eyes; Propotosis; Cataract; Nystagmus; Any Scars
b. Feel:
Measure the head circumference
Sutures- Widening; Overlapping
Fontanelles (Ant and Posterior)- Tension ?bulging
Consistency of skull
VP Shunt
c. Auscultate for cranial bruits - Note any facial dysmorphism
- Examine hands for anomaly
- Access one’s neurological and developmental status
