Abdominal Aortic Aneurysm Flashcards
what is the screening process for AAA?
single abdominal ultrasound for males aged 65
screening outcome
<3 cm
normal
no further action
screening outcome
3 - 4.4 cm
small aneurysm
rescan every 12 months
screening outcome
4.5 - 5.4 cm
medium aneurysm
rescan every 3 months
screening outcome
> /= 5.5 cm
large aneurysm
refer within 2 weeks to vascular surgery for intervention
what AAAs are low rupture risk?
- asymptomatic
- aortic diameter <5.5 cm
what AAAs are high rupture risk?
- symptomatic
- aortic diameter >/= 5.5 cm
- rapidly enlarging (>1cm/year)
what is the management of high rupture risk AAA?
- elective endovascular repair (EVAR)
- open repair
what is classed as a rapidly enlarging AAA?
> 1cm / year
what is elective endovascular repair?
- sent placed in the abdominal aorta via the femoral artery
- prevents blood from collecting in the aneurysm
what is a complication of EVAR?
endo-leak
stent fails to exclude blood from the aneurysm
what are the features of ruptured AAA?
- severe, central abdominal pain radiating to the back
- pulsatile, expansile mass in the abdomen
- shock
- collapse
what is the management of suspected ruptured AAA?
immediate vascular review
suspected ruptured AAA + haemodynamically unstable
straight to theatre
suspected ruptured AAA + haemodynamically stable
CT angiogram
