Referred pain from the parietal peritoneum is somatic and is usually severe, so its site can be localized. The anatomical basis for this localization iof pain is that the parietal peritoneum is supplied by somatic sensory fibers through thoracic nerves, while visceral referred pain from an organ would be through greater or lesser splanchnic nerves. Inflamed parietal peritoneum is extremely sensitive to stretching. When digital pressure is applied to the anterolateral abdominal wall over the site of inflammation, the parietal peritoneum is stretched. When the fingers are suddenly removed, extreme localized pain is usually felt, known as rebound tenderness.
