To complete the assessment I will perform
One set of vital signs
Full lymph node assessment
Rectal digital assessment
Dear prof, Mr Tan is a xx years old gentleman come in because complaining of vomiting blood/abdo pain/vomiting I perform an abdominal assessment for him.
Upon inspection, there is no evidence of distressed or cup
There is an evidence of vomitus in the cup
Mr Tan doesn’t not appear jaundice,
no clubbing found in fingers,
Skin over the chest and abdomen are intact
no evidence of spider naevi, caput Medusa or chest insertion mark
No gynecomastia
No abnormal breath found
Upon palpation
No palpable mass are found over the 9 quadrants
Lymph node is not enlarged
Percussion The size of the liver
Is in normal range of 6-12cm
Tympanic sound heard over the percussion on spleen
No evidence of fluid accumulation in the abdomen
Which indicates normal findings
Upon auscultation
Bowel sound is heard over the four quadrant with
No renal or aortic bruit heard
Renal punch is negative,
No evidence of edema over the lower limbs and sacral
Overall the assessment is consistent with a normal finding
