Inpatient levels of care
A
inpatient refers to a facility where the patient sleeps there
- acute care
- acute inpatient rehab
- subacute inpatient rehab/transitional care unit (TCU)
- skilled nursing facility (SNF)

Acute Care
A
- Who: acute medical need (not medically stable); after surgery, CVA, TBI, SCI, Medical Illness (infection, pneumonia, trauma, some fractures, CA, some acute cardiac conditions)
- What patient’s receive: Medical care. PT Consult and treatment (tx) pro re nata (PRN)/as needed
- Length of stay: as short as possible until medically stable. Really expensive
- Goals of PT: Determine disposition, decrease risk of complications

Inpatient acute rehab
A
- Who: medically stable, but not able to return home (after acute illness); after surgery, CVA, TBI, SCI, Medical Illness (infection, pneumonia, trauma, some fractures, CA, some acute cardiac conditions)
- What patient’s receive: 3 hrs of therapy daily PT, OT, and speech therapy (PRN), usually 7 days a week
- Length of stay: varies, ~1-3 weeks or longer (AVG = 21 days)
- Goals of PT: increase functional mobility to allow d/c home

TCU/Sub-acute
A
- Who: medically stable, but not able to return home (same list as acute care); generally less complex
- What patient’s receive: PT, OT, ST if needed; 1-2 hrs a day usually 7 days/week
- Length of stay: varies, ~1-2 weeks or longer
- Goals of PT: increase functional mobility to allow d/c home

SNF
A
Skilled Nursing Facility
- Who: medically stable, but not able to return home (same list as acute care); with significant comorbidities
- What patient’s receive: PT, OT, ST if needed; 30-60 minutes of therapy a day usually 5-7 days/week
- Length of stay: varies, ~20 days, up to 90
- Goals of PT: increase functional mobility to allow d/c home or LTC (long term care) for chronic conditions
