influenza
- fluzone: for pts >65 (contains 4x antigen amount)
- fluad: for pts ?65 (standard dose)
does CDC recommend 1 influenza vaccine over another?
no they do not
pneumovax (PPSV23)
all adults 65 and older should receive 1 dose
high risk patients- PPSV23
- may need to receive 1 dose under 65 yo then need to get ONLY ONE MORE at 65+
- must wait 5 YEARS btwn doses
prevnar (PCV13)
shared clinical decision making for healthy seniors WITHOUT immunocompromising condition, cerebrospinal fluid leaks or cochlear implant
spacing for PCV13 & PPSV23
- pneumo-naive: 1 PCV13 at age 65 then PPSV23 1 year later
- not pneumo-naive: give PCV13 at 65, then PPSV23 1 year later (at least 5 years after the first)
- if pt received PPSV23 at or after 65, give PCV13 1 year after
zostavax (ZVL)
- for pts 60 and older
- liver attenuated vaccine
shingrix (RZV)
- preferred!!
- for pts 50 & older
- 2 IM doses given 2-6 months apart
Tdap/Td
-1 IM dose of Tdap for all adults then 1 dose of Tdap or Td every 10 years after that
hepatitis C screening

bone health screening
- women: all 65 and older
- men: all 70 and older
- monitor every 2 years
risk for bone health
high risk: post menopausal women & men aged 50-69 yo, low body weight, prior fracture, high risk meds
daily vitamin D requirements
- younger than 50: 400-800 IU
- adults over 80: 800-1000 IU
daily calcium requirements
- 1000 mg for women 50 & younger and men 70 & younger
- 1200 mg for women 51 & older and men 71 & older
calcium carbonate
most common form
