7] Total Joint Arthroplasty Flashcards
What’s TJA fixation
Cemented use in older population b/c it allows for higher load after surgery
Mechanical causes
Wear and stress
Biological causes of TJA failures
Infection
Bone restoration
Aseptic loosening
Surgical reasons for TJA failure
Poor fit
Dislocation
TJAs last? And for who?
They last 10-15 years so better for ppl over 70 years old.
Less than 55 years 3x more likely to do it again.
Indications for TJA surgery
Hip pain
Stiffness
Failure of conservative txt
3 types of surgical procedures
Anterolateral approach
Posterolateral approach
Anterior approach (minimally invasive)
Anterolateral approach
C shaped cut on back of greater trochanter
Space for anterolateral approach
Between glute med and TFL
Posterolateral approach
Skin cut over greater troch with distal straight limb and prox limb curved posteriorly
Access space for posterolateral approach
Between glute max and med
Anterior approach
Special table to do rotation and traction.
Incision made distal and slightly lateral to ASIS
Space for anterior approach
Between TFL, sartorius and rectus femoris
What’s opened and compromised in anterior approach?
Anterior capsule is opened.
No muscles are compromised.
Complications of TJA
Nerve injury
DVT
Dislocation
Leg length discrepancy
