In skilled nurshing homes and extended care facilities not operating a pharmacy, the drug or medicine room shall be under the direct supervision of
a consulting pharmacist or a member of the medical staff who shall be a licensed practitioner of medicine
in nursing homes not classified by the board of health as skilled nursing homes, maternity homes, homes for the aged, domiciliary institutions, and all related institutions, Rx only drugs shall be furnished by a
licensed pharmacy on the prescription of a licensed practitioner of medicine for INDIVIDUAL patients and there shall be no Rx legend drugs on the premises of these institutions other than those prescribed (except emergency kits)
For a facility that does not have an institutional pharmacy, drugs may be stored in a stat cabinet to which only authorized personnel may obtain access by key, combination, or access code given
- contents consist of non-controlled drugs that are not available from another source in sufficient time to prevent harm
- each facility may maintain 1 stat cabinet for purpose of mimum samount of stock meds needed after duty
- there shall be records available to show the amount received, name of resident, and amount used, physician, time of admin, name of individual removing, and using the medication
- the pharmacist shall inspect the cabinet at least montly replacing outdated drugs
how often should a pharmacist check the stat cabinet used after hours in long-term care facilities
at least monthly
What should be recorded when an individual takes meds out of the stat cabinet after hours
- amount taken
- name of resident
- amount used
- physician
- time of administration
- name of the individual removing
- name of patient
whenever any drug is not available from the floor suppliers or cabinet, and such drug is required to treat immediate needs of a patient whose health is otherwie jeapordized, such a drug may be obtained from
the pharmacy; 1 supervisory nurse or physician in any given shift is obtaining drugs from the pharmacy
Unit dose medications shall be packaged according to an acceptable format to include
- product name
- strength
- control number
- expiration date
The contents of all individual prescriptions shall be kept in
original dispensed containers bearing the original prescription label
all containers of medicines and drugs shall be properly and plainly labeled, including the
- name of the drug
- strength
- resident’s name
- physician
- date of filling
- directions for administration
- prescription number
- experiation date
- number of pills
- auxiliary laels
a managing pharmacy may use an automated dispensing system to meet the emergency medical needs and stat medication needs of residents; the automated dispensing system must be located in a
skilled nursing facility that holds a valid and current contract with a managing pharmacy to provide services to that facility
Access to automated dispensing systems shall be controlled by the
managing pharmacy
access to the automated dispensing system shall be limited to
licensed nurses, pharmacists, registered technicians, and authorized field service personnel for maintenance purposes and only while under supervision of a nurce, pharmacist, or tech
for a facility that does not have a pharmacy onsite, drugs may be provided for use by authorized personnel in emergency kits stored in secured areas to prevent
unauthorized access
the exterior of each emergency kit sshall be labeled so as to clearly indicate that it is an emergency drug kit and that it is for emergencies only. The label shall contain a list of
all the drugs in the kit, including
1. name
2. strength
3. quantity
4. expiration
5. name of the pharmacist
6. address of the pharmacist
7. telephone of the pharmacist
Drugs shall be removed from emergency kits only pursuant to a valid written order of an authorized practitioner. When a kit is opened, the pharmacist should be notified of this, and will stock and reseal within
no more than 72 hours
