grand multip [over distended uterus/abdomen] malposition malformation (hydrocephaly, etc) polyhydramnios iatrogenic PROM pelvic contracture
Treatment for cord prolapse
push head back in
C section
Cord prolapse etiology
long cord, excess fluid monoamniotic twins fetal movement (Males!)
Uterine rupture caused by…
induction by pitocin is biggest risk
also shoulder dystocia
vertical C sxn scar
transverse scars
“bikini cut”, more likely to dehisce, increasing risk of hemorrhage
Risk factors for uterine rupture
perforation of uterus
bad TAB
distention (multip, polyhydramnios)
any uterine surgery (related to or unrelated to preg)
s/sxs uterine rupture
sudden decrease FHR!
if upper: sharp, tearing pain; syncope; abn vaginal bleeding; rigid abdomen
if lower: sublte, ache, UCs become weaker, fetal distress
Management of uterine rupture
left lateral decubitus, oxygen, tx shock, abx, uterine repair
may need hysterectomy
