A
term infant (head calcified!) vertex pres engagement to +3 station ruptured membranes full cervical dilation min CPD

A
persistent OP
asynclitic vertex
exhausted mother
fetal distress

Maternal complications
A
vaginal lacerations from improperly applied cup

Fetal complications
A
scalp abrasions, bruising, hematoma, jaundice, low APGAR, cranial fracture, death

Procedure
A
assess engagement, mom voids apply cup to fetal head, pump to 10 mmHg check to ensure no vaginal tissue stuck increase P to 40-60mmHg traction reduce P to 10 mmHg repeat with each UC

Precautions
A
need continual fetal monitoring
avoid getting blood/fluid in pump

Indications to discontinue use
A
2 pop offs
>30 mins total vacuum use
10 mins during UCs
