7. Arsenic poisoning Flashcards
What are the sources of arsenic poisoning?
Widely used in industrial processes, manufacture of:
- semiconductors
- wood preservatives
- glass
- herbicides
- released during the burning of coal,
- “magic bullet” for syphilis – first pharmaceutical antimicrobials.
What is Pharmacodynamics/MOA of arsenic?
Inhibition of enzymes via sulfhydryl binding.
What are the symptoms of chronic arsenic poisoning?
- skin changes (stocking-glove pattern)
- hair loss
- bone marrow depression, anemia
- chronic nausea and gastrointestinal disturbances
- carcinogen.
What is the major route of absorption of arsenic?
Respiratory and GI tract
What are the symptoms of acute arsenic poisoning?
- Severe gastrointestinal discomfort
- vomiting
- “rice-water” stools
- capillary damage - dehydration and shock.
??
Arsine gas industry (manufacture of semiconductors) Causes massive hemolysis – Pigment overload from erythrocyte breakdown can cause renal failure. Treatment is supportive - exchange transfusion, vigorous hydration.
How is the metabolism and elimination of arsenic in the body?
Metabolism: methylation, mainly in the liver.
Elimination: via kidney
How do you treat chronic arsenic poisoning?
Dimercaprol therapy appears to be of value.
How do you treat acute arsenic poisoning?
Replace water, electrolytes, and chelation therapy with dimercaprol
