Define continuing care?
Critical link between treatment and continue with recovery
Define assessment
Procedures by which counsel/programs identifies and evaluate a persons strength, weakness, problems and needs for the development of the treatment plan
Define Treatment planning
Process by which the counselor and the inpatient identify and rank problems needing resolution
Define strength
Inherent qualities, behaviors and beliefs that are likely to support and enhance treatment and recovery
Define weaknesses
Inherit characteristics, behaviors, and or beliefs that are apt to sabotage, Block, or be a liability to treatment objectives and recovery
What is the purpose of continuing care plans?
- Provide critical link between initial, current, and follow on care
- Provide ongoing support to continue work on issues
- Provide ongoing feedback in a therapeutic setting supporting new behaviors and lifestyle changes
- Identify warning signs of relapse and help minimize potential relapse incidents
What is the scope of continuing care plans
- Relapse prevention and ongoing recovery
