How common is it?
1% of >65s have heart failure
What causes it?
Coronary artery disease –> scarring
HTN (esp. if poorly controlled) –> hypertrophy
Arrhythmias, cardiomyopathies
What risk factors are there?
Diabetes
Smoking
Obesity
Sedentary lifestyle

How does it present?
SOB
Orthopnoea
PND
Symptoms of volume overload
What signs may the patient have on examination?
Crackles / decreased breath sounds on lung auscultation Raised JVP Positive hepatojugular reflux Low bp, tachycardia Oedema
What other conditions might present similarly?
Pulmonary hypertension
COPD
Anaemia
Venous insufficiency / DVT (lower limb oedema)
How would you investigate?
Troponin, FBC, LFTs, U+Es
ECG, BNP
Coronary angiogram
Echo
What treatments are there?
Lifestyle modification
Loop diuretics, ACEis, B blockers
Meralocorticoid receptor antagonists eg spironolactone, digoxin, vasodilators
What complications are there?
Respiratory distress
Arrhythmias
Cardiogenic shock
Sudden death
