Who does it affect?
All ages
Most common >65s
What causes it?
Raised peripheral resistance
Prolonged smooth muscle contraction in small arterioles leads to structural changes eg thickening of vessel walls
Causing irreversible rise in PR
How does it present?
Usually asymptomatic
Headaches, fatigue, vision problems
What signs may the patient have on examination?
Irregular heartbeat
Haematuria
What other conditions might present similarly?
CKD
Renal artery stenosis
Pre-eclampsia
Coarctation of aorta
How would you investigate?
Taking bp reading
How would you investigate?
Taking bp reading in both arms - record lowest reading
ABPM
What treatments are there?
ACEis, A2RBs, beta blockers, calcium channel blockers, diuretics
What complications are there?
MI Stroke Heart failure Peripheral vascular disease Renal disease AAs Vascular dementia Retinopathy
