6. Chronic Kidney Disease Flashcards
What is Chronic Kidney Disease?
Any Long-term reduction in Kidney Function
- eGFR < 60 for 3+ months
How many adults have CKD in the UK?
10% of Adults
What are the most common causes of CKD in London?
- Diabetes
- Glomerulonephritis
- Hypertension
- Polycystic Kidney Disease
- Pyelonephritis
How do we measure GFR?
24 Hour Creatinine Clearance (Pregnancy)
Isotopic Methods (Radioisotopes)
What is the best way to calculate GFR?
eGFR
How many stages of CKD are there?
5
What can you see in Stage 1 of CKD?
Any kidney issue but eGFR over 90
What can you see in Stage 2 of CKD?
Any kidney issue but eGFR between 60 and 90
What can you see in Stage 3 of CKD?
eGFR between 30 and 60
What can you see in Stage 4 of CKD?
eGFR between 15 and 30
What can you see in Stage 5 of CKD?
eGFR less than 15
Give 6 roles of the Kidney
- Removes Fluid/Sodium
- Controls serum pH
- Controls serum K
- Regulates BP
- Regulates Hb via EPO production
- Controls Bone and Mineral Metabolism
- Excretion of Ca/PO and through Vitamin D
What happens in CKD?
- Fluid Overload/Oedema
- Metabolic Acidosis
- Hyperkalemia
- Hypertension
- Anaemia
- Hyperphosphatemia, Hyperparathyroidism, Hypocalcemia
What is the main priority in treating CKD?
Blood Pressure
- It tends to rise in CKD and High BP can make it get worse faster
What is the SECOND priority in treating CKD?
Reduce Proteinuria
- Lowering BP improves proteinuria
- ACEI also reduce proteinuria directly as do ARBs
