Provocation/Special Tests Palpate for…

A

AC joint- sprain or DJD
SC joint- sprain
Supraspinatus Tendon- tendonitis (flat broad region in from of accordion process). Health= feel nothing
Infraspinatus tendon- tendonitis, sitting 90° flex, horrid add, and ER. Find post/late corner of accordion and drop inferiorly 1cm.
Biceps tendon- tendonitis

Neer (Overhead) Impingement Test

A
Sitting: stabilize spine of scap to prevent post tipping & trunk extension. Passively IR shoulder and elevate/flex in plane of scap. End-range over-pressure reproduces pain as subacromial structures are compressed.
Thumb down (IR)= surpaspinatus
Thumb up (ER)= long head of biceps

HAwkins-Kennedy Impingement Test

A

Sitting, elevate shoulder to 90° in plane of scap. Perform passive medial rotation to end range. Pain at overpressure indicates compression of supraspinatus against acromion???

A

Same as Hawkins-Kennedy but performed with horizontal adduction across midline.
Test of lesser tubercle impinging against medial coracoid process.
