Malignant prolif of lymphocytes in blood/BM
Many disorders; CLL most important
ONLY in adults
Long course; indolent but incurable
What are the 4 things you must know about Chronic Lymphocytic Leukemia?
Small, mature lymphocytes
= small lymphocytic lymphoma
B-cell, but CD5+ (a T-cell marker, weird)
Long but inexorable course
What might you see in a bone marrow section of a patient w/ CLL?
LYMPHATIC NODULES
What is the definitive, “gold-standard test,” in CLL?
Immunophenotyping
What are the results of immunophenotyping CLL?
B-cell Ag present (CD19 and/or CD20)
CD 5 present (a T-cell Ag, indicates less mature B-cell)
TdT NOT present (enzyme you’d see in lymphoblasts)
Why would you see hypogammaglobulinemia in CLL?
B/c normal Ab production is likely inhibited (B-cells are fucked up)
What is the genetic pathology in CLL?
Bcl-2 gene rearrangement –> ↑Bcl-2 (prevents apoptosis)
Thus, B-cells just kinda accumulate
What do patient’s w/ CLL usually die from?
Infxn (makes sense w/ hypogammaglobulinemia)
WHAT ARE THE 4 things you must know about Hairy Cell Leukemia?
Hairy cells
Splenomegally w/o lymphadenopathy
Pancytophenia (due to fibrotic BM)
TRAP + (special chemical stain)
Who gets HCL more, ♂ or ♀?
♂, at a 5:1 ratio!
What are the results of Immunophenotyping of HCL?
B-cell Ag’s CD25 & CD11c present (VERY specific for HCL)
CD5 NOT present
What are the 4 things you must know about Prolymphocytic Leukemia?
↑Prolymphocytes
Splenomegaly w/o lymphadenopathy
Rare
Aggressive
What are the 4 things you must know about Large Granulated Lymphocyte Leukemia?
↑Large granulated lymphocytes
T-cells!!!!!!!!!!!!!!!!!
Neutropenia
Long survival
