Confirmation
Matter: oil of Chrism, and the laying on of hands
Form: “Be sealed with the Holy Spirit”
Eucharist
Matter: bread and wine
Form: “This is my body, this is my blood”; Prayer of Consecration says this, “do this in memory of me”
