48 - DKA Flashcards
DKA is caused by..
insulin deficiency
hyperglycemia
DKA is characterized by
-hyperglycemia
-ketosis
-acidosis
-dehydration
DKA precipitating factors
-illness
-infection
-inadequate insulin dosage
-undiagnsoed DMT1
-neglect
Ketones
compensatory breakdown of fat
-acidic
Insulin deficiency on protein synthesis
-impairs protein synth
-»»>.leads to nitrogen losses fr tissue
-production of glucose fr proteins + liver
»»more hyperglycemia
»>adds to osmotic diuresis
insulin deficiency on electrolyte imbalance
-depletion
Na, K, Cl, Mgm phosphate
dehydration in DKA
-dry mucous membranes
-tachycardia
-orthostatic hypotension
early s/s
lethargy + weakness
late signs of dehydration
-skin is dry + loose
-eyes soft + sunken
-ab pain w anorexia, n/v
kussmaul resp
rapid deep + dyspnea
-attempt to reverse metab acidosis
diagnostics for DKA
BLOOD GLUC
pH
SERUM BICARB
URINE
BLOOD GLUC - >250mg/dL
pH - <7.30
SERUM BICARB - <16mEq/L
URINE - mod to large ketones present
if DKA w PNA + UTI…
then admit to hospital
first goal of therapy
establish IV access
-admin fluid + electrolyte replacement
preferred fluids for IV
0.45-0.9 NaCl
when blood gluc is approaching 250…
add 5-10% dextrose
-to prevent hypoglycemia
