- disclosure of any active or inactive license
- payment for the exact amount of required fees

(f) Applicant is not required to provide proof of transcripts if previously licensed in Oregon

Individual Qualifying for Licensure Through Reciprocity:
(a) Submit an affidavit of licensure demonstrating proof of holding a current active license as a tattoo artist. Must be equivalent to Oregon licensing requirements or the applicant must have been working as a tattoo artist with the equivalent of THREE YEARS of experience of the last five years, or FIVE YEARS out of the last ten years.
(b) Pay an examination fee
(c) Submit a passing score of the exam within two years of application
(d) Pay required license fee
