planning sessions and performance evaluations are conducted annually on
all sworn members to provide structure and guidance in developing performance capabilities and career opportunities
the planning session and the performance evaluation are documented on
sapd form 6-pe
performance evaluations will be submitted to the reviewing officer by
the 15th of february
probationary officers are evaluated
weekly while with they are with a fto and then monthly for the remainder of there probation
rater training will be on the following

members may document in part IV of sapd form 6-pe any comments they might have regarding their annual performance evaluation. if additional space is necessary they can use
sapd form 65 the interoffice correspondence is used and becomes part of the evaluation form
upon completion of the evaluation the members supervisor submits it to the reviewing officer
if there is disagreement between the reviewing officer and the rater which can not be resolved the reviewing officer records his assessment of the rating in
red ink next to the raters rating
upon completion and supervisory review of a sworn members annual performance evaluation form the supervisor
conducts a formal performance appraisal counseling and coaching interview with the sworn member
if an agreement can not be reached between the supervisor and the sworn member the rating maybe appealed to the
reviewing officer.
appeal must be in writing using sapd form 6-nar and submitted with in 5 working days
the reviewing officer has 10 days to investigate the appeal and respond
reviewing officer shall route all completed original annual performance evaluations forms to the manpower allocation detail by
february 28
performance evaluations are kept in the accounting and personnel officer for a period of
3 years
the only personnel who have access to the performance evaluations are
the sworn member the supervisor conducting the eval the reviewing officer manpower allocation detail the training advisory board accounting and personnel office
