What is eczema herpeticum?
A rare but severe skin infection that occurs at sites of skin damage caused by the herpes simplex virus
What HSV types can causes eczema herpeticum?
HSV 1 or 2
How can eczema herpeticum sometimes occur?
As a cold sore
What can cause the skin damage leading up to eczema herpeticum?
- Atopic dermatitis (eczema)
- Burns
- Long use of steroids
- Cosmetic procedures
What cosmetic procedures can predispose to eczema herpeticum infection?
- Lasers
- Skin peelers
- Dermabrasion
How does eczema herpeticum often start?
As a group of small blisters in areas of skin damage and then spreading to healthy areas of skin
Describe the blisters seen in eczema herpeticum?
Painful, pus or fluid filled and can bleed, ooze or weep then crust over
What systemic symptoms can be caused by eczema herpeticum?
- Pyrexia
- Rigors
- Lymphadenopathy
How is eczema herpeticum often diagnosed?
Clinically
What can be used to confirm eczema herpeticum infection?
Swabs of the blisters
What are the differentials for eczema herpeticum?
- Chickenpox
- Impetigo
- Eczema flare up
What should be given to treat eczema herpeticum?
Anti-viral e.g. aciclovir
How can aciclovir be administered in eczema herpeticum?
Orally or IV if not possible
What may be needed in addition to antiviral in eczema herpeticum?
Antibiotic to cover secondary bacterial infection
What doe severe cases of eczema herpeticum require?
Hospital admission
