Q

Q 4. What two neurotransmitters show level change in Alzheimer disease? What change for each?

Q

Q 9. What is course of alcohol withdrawal symptoms

  • mild symptoms (3)
  • severe symptoms after prolonged withdrawal (2)
A

within first 6-24hrs
-insomnia, tremor, anxiety

after 2-3 days
- derlium, hallucination

  • like BDZ, alcohol works on GABA as a DEPRESSENT
for withdrawal, it will be stimulant effect. so insomnia, tremor, and anxiety

Q

Q 17. Farmer exposed to organophosphate. What does it do in terms of muscle?

A

organophosphate -> AchE inhibitor
-> excess nicotinic stimulation at NMJ -> muscle fasicculation and subsequent paralysis

  • also BB SLUDGE

Q

Q 17. What are two antidote for organophosphate intoxication? what is MOA & effectiveness of each? which one is first line why?

A
  • atropine: MUSCARINIC receptor inhibitor. ONLY reverses BB SLUDGE
=> atropine alone can not reverse muscle paralysis as it does not inhibit nicotinic receptor
  • pralidoxime: regenerate AchE
=> can reverse BOTH muscarinic and nicotinic stimultion
  • atropine is the first line because pralidoxime is effective ONLY IF GIVEN EARLY

Q

Q 20. Entacapone for Parkinson’s disease: what is targeted enzyme. This enzyme mediates what exact reaction?

A

COMT

levodopa –> 3-o-methyldopa

17
Q

Q 22. erosion vs. ulcer: difference in terms of penetration?

A

erosion: penetrating through muscularis mucosa, but not fully penetrating it
ulcer: complete penetration through muscularis mucosa, involving submucosa

18
Q

What are three components of mucosa

A
  • epithelium
  • lamina propria
  • muscularis mucosa
19
Q

Q 24. What is role of fibroblast in atherloscelrosis formation?

A

fibroblasts are not associated with atherosclerosis formation.
It is required for scar formation

20
Q

Q 24. What mediates formation of fibrous cap in atherosclersosis formation?

A

vascular smooth muscle

21
Q

Q 26. Thrombosis of what vein results in gastric varicies only at gastric fundus

A

gastric vein

22
Q

Q 32. Patient with extensive hemorrhage. How does it lead to what type of renal injury?

A

acute tubular necrosis

23
Q

Q 32. Describe three phase of acute tubular necrosis

A
  • initiation: intiatino of injury
  • maintenance phase: oliguria, maintenance of low GFR, risk for hyperkalemia
  • recovery phase: polyuria. regeneration of epithelial tubes. polyuria (quick recovery of GFR), risk for hypokalemia (rapid adjustment for hypokalemia, while tubule is not fully regenerated for full function)
24
Q

Q 33. Histology of metastatic cancer shows white/clear cytoplasm. What is most likely origin of cancer?

A

renal cell carcinoma

  • buzzword: clear cytoplasm
25
Q

Q 35. Three phenotypes of Chediak-Higashi syndrome?

A
  • partial albinism: light skin, silvery hair
  • recurrent infection
  • peripheral neuropathy: nystagmus
26
Q

Q 37. Cognition test: what is way to test concentration?

A

reciting months of year backwards

  • you need to keep concentrating to tell it backward. with impaired concentration, you cannot do this
27
Q

Q 37. Cognition test: what is way to test comprehension

A

following multiple commands

  • you need to understand to follow commands. this is NOTHING to do with concentration
