294 ARDS Flashcards
Clinical disorders associated with direct lung injury leading to ARDS
Pneumonia Aspiration of gastric contents Pulmonary contusion Near drowning Toxic inhalation injury
PANT
Clinical disorders associated with Indirect lung injury
Sepsis Severe trauma Multiple transfusion Drug overdose Pancreatitis Post cardiopulmonary bypass
PS MD
Causes 40-60% of ARDS
Pneumonia and sepsis
Among patients with trauma, most reported surgical conditions in ARDS?
Pulmonary contusion
Multiple bone fractures
Chest wall trauma/flail chest
Apache score associated with 2.5 fold increase risk of developing ARDS
Apache score of more than 16
Phases of ARDS
Exudative
Proliferative
Fibrotic
Phase of ARDS. Endothelial cells and type I pneumocytes are injured. Edema. Fluid rich in protein accumulates in interstitial and alveolar spaces.
Exudative phase
Involved areas of alveolar edema in ARDS
Dependent portions of the lungs with diminished aeration
Severity of ARDS.
Mild. PF less than 300
Moderated. PF less than 200
Severe. PF less than 100
Chest xray findings consistent with ARDS
Bilateral opacities involving at least 3/4 of lung fields
