24hr PC Flashcards
24 hour postural care
a therapeutic and preventative approach that
considers body positions over a 24-hour period in
order to maximize function and decrease body
distortion throughout the day and over time
assessment of all positions used across 24 hrs
implementation of intervention to improve symmetry in order to protect body shape an dpromote functino
goals of 24 hr PC
Help to mitigate the impact of gravity on body shape
Decrease pain
Improve sleep
Increase ease of care
Improve digestion, respiration, and toileting
Improve sitting posture and function during the day

identify:
GMFCS
known patterns of distortion
mansfield checklist
possible diagnoses: NM disorder CP, muscular dystrophy
chornic pain
dementia
identify predictable patterns of distortion
CP can cause postural asymm, increases change of body distortion, higher GMFCS levels = greater risk for asymm
severe postural asymm incrased the likelihood of
- scoliosis 9x
- windswept hips 6-9x
- hip and knee flexion contractures 7-12x
hip displacement
1 in 3 children w CP
greater risk with higher GMFCS level
level 5 = 68-90% chance
mansfield checklist
Does their body stay in a limited number of positions?
* Do their knees seem drawn to one side?
* Are their arms in a position in which it is difficult to use them?
* Does their head seem to turn mainly to one side?
* Does their body bend forward/backward/sideways?
* Is their body already asymmetric?
If you have answered yes to any of these questions,
the person you are thinking of may benefit from postural care

Analyze routines
* Match positioning support to goal of task
* Consider the number of transitions
throughout the day
* Determine starting schedule for lying support
* Train all staff members and family

Short term or day to day feedback
* Understanding barriers
* Cheerleading and celebrating success
