What are features of Autoimmune hepatitis?
Type 1- affects young women
ANA antibodies
Anti smooth muscle antibodies
IgG positive
What is the treatment of recurrent C diff infection?
Within 12wks of previous infection= fidaxomicin
>12 weeks post infection= vancomycin or fidaxomicin
How can IBS and IBD be differentiated in primary care?
Faecal calprotectin
What is the management of mild- moderate flare of UC?
Proctitis
- rectal mesalazine
- if no improvement within 4 wks then add oral
- if no improvement still then add oral prednisolone
Proctosigmoiditis and left sided UC
- topical mesalazine
- if remission not achieved then consider either stopping topical and adding oral mesalazine and topical steroid OR just add high dose oral mesalazine to topical
- if still no improvement then switch to oral mesalazine and prednisolone
Extensive disease
- oral and topical mesalazine
- if no improvement then stop topical treatment and add prednisolone
What is the management of a severe flare of UC?
IV steroids
If contraindicated then IV ciclosporin
If no improvement after 72hrs on IV steroid then add IV ciclosporin
How can the severity of C diff infection be determined?
By the WCC count
<15= moderate infection
>15= severe infection
Normal= mild infection
What are key features of achalasia?
Difficulty swallowing both solids and liquids- non progressive
Heartburn
Regurgitation
What is used first line to maintain remission in Crohns disease?
Azathioprine or mercaptopurine
What should be avoided in bowel obstruction?
Metoclopramide
What are causes of C diff infection?
Clindaymcin
Ceftriaxone
Cefotaxime
Cefuroxime
What is the best marker of acute liver failure?
Raised prothrombin time
Decreased albumin
What are features of Budd Chiari Syndrome?
Ascites
Abdo pain
