19. Exam for Lower Extremity (9/14) Flashcards
Inability to bear any weight r/o
fracture or dislocation
During a basic hip exam, referred pain can present from:
from sacroiliac (SI) joint, back or knee
Adduction ≥ 30° of leg test for what muscles:
Adductor longus, A brevis, A magnus,
Gracilis
Abduction ≥ 45° of leg test for what muscles:
Gluteus medius, G minimus, Tensor
fascia latae
Flexion ≥ 110°
Iliopsoas, Rectus femoris, Sartorius
Extension ≥ 10°
G maximus, Biceps femoris,
Semimembranosus, Semitendinosus
Internal Rotation (medial rotation )≥ 30°
A longus, A brevis, A magnus, Tensor
fasciae latae, G medius, G minimus
External Rotation(lateral rotation) ≥ 40°
G maximus, Piriformis, Gemellus
superior Obturator internus, G inferior,
Quadratus femoris, O externus
Functional motion
a) Flexion _____
ER & IR ____
Abduction ____
(120°)
(20°)
(30°)
Loss of which three motions in lower extremity for functional motion?
rotation, flexion & abduction
Appearance of leg check for:
symmetry & leg length
On leg exam be sure to palpate:
- Iliac crest
2. Greater trochanteric region
Referred pain in knee may come from:
from back or hip
When checking the appearance of the knee be sure to check for:
symmetry & effusion
Popeye deformities .
Muscle wasting
Prepatellar bursa
When palpating the leg (in particular for knee) check for:
Effusion
Quadriceps & patellar tendons
Tibial tubercle
Joint line
Range-of-motion expectations for knee:
Flexion
extension/flexion
≥ 130°
0-110°
Ankle & foot
X-rays for what types of situations?
a) Bony point tenderness at posterior edge ± tip of either malleolus
(1) ) Probably can ignore tip pain
b) Bony point tenderness at navicular and base of 5th metatarsal
c) Inability to walk (4+ steps) both immediately after the injury and in the ER
When palpating the ankle at the proximal fibula, what are we checking for?
What other exam can we do to see if this is the issue?
Proximal fibula – Maisonneuve fracture
Squeeze test ilicits pain at knee
Squeeze test to rule out what two clinical findings?
fibular head fracture (Maisonneuve fracture) and
syndesmotic/high ankle sprain.
