things that make urine look cloudy
bacteria, blood, sperm, crystals, mucus
leukocyte esterase in urinalysis
detects esterase released by WBCs, suggests infection
nitrite in urinalysis
bacteria convert nitrates > nitrites
not all bacteria convert it
urine dipstick protein indicator detects
albumin - WONT catch bence jones protein, ig, etc
SSA test
sulfosalicylic acid test
detects more proteins than just albumin
ACR
albumin creatinine ratio, used to quantify proteinuria relative to Cr clearance/GFR
blood on urinalysis
doesn’t measure Hgb - measure pigment, so could also be myoglobin
nl urine specific gravity
1.002-1.035
what does high specific gravity for urine indicate?
contamination, glucosuria, radiodye
hyaline casts
casts of Tamm-Horsfall mucoprotein from inside tubules
RBC casts make you think
glomerulonephritis
WBC casts make you think
interstitial nephritis, pyelonephritis, lupus nephritis
tubular/muddy brown casts
seen in ATN
fat/waxy casts
indicate chronic/advanced kidney dz
causes of acute interstitial nephritis
70% drug hypersensitivity (abx, sulfas, NSAIDs, allopurinol)
15% infection (strep, legionella, CMV, etc)
8% idiopathic
6% autoimmune dz
