- How do you detect contrast induced kidney injury?
Serum creatinine rise>26 micromoles within 48 hours
Serum ceratinine rise>1.5 fold from baseline whtin one week post scan
Urine output<0.5ml/k hour for 6 hours
What are the risk factors for an AKI?
Hpovolemia?dehydration CKD/AKI Heart failure Renal transplant Age>75 Repeated contrast administration/several examiantions
How do you assess risk for contrast kidney problems?
EGFR- levels past 3 month (stable)
Questionaiire for under70’s (diabetes, BP, HF, Kidney trouble)
What groups of peopel need special treatment pre CT
No metformin
Thyroid function of baby 1 week post delivery if had scan
Lactation- no action required
What sort of reactions may occur due to contrast reaction?
Nausea/vomtiing Urticaria Bronchospasm Laryngeal oedema Hypotension Generalised anaphylaxis
What is contrast extravasation
When the contrast doesnt go into the vein and instead goes into the tissue.
Causes swollen limb, pain and cyanosis of the limb
Contact plastic surgery
What procedures are CT scans used in?
Head trauma General trauma Acute surgical abdomen Post-operative surgical abdomen Aortic dissection Stroke/intracranial hemorrhage PE Cancer detection and staging
What should you mention when referring a patient for a CT
Presenting complaint PMH Allergies Renal function Hyperactive/delerius Previous imaging What clinical question will this answer What will you do differently
