14 Flashcards
1
Q
why do biopsy
A
- diagnosis
- guide treatment
- affect prognosis?
2
Q
when to do excisional biopsy
A
remove entire visible mass
* benign masses
* some gingival masses
* treatment not altered by tumor type
* re-excision possible without great morbidity- BEST CHANCE OF CURE IS 1st SURGERY
3
Q
when to do incisional biopsy
A
- large superficial lesion
- morbidity associated with definitive surgery without a diagnosis (liver)
- careful planning needed based on size and location
- less invasive sampling has not yielded a diagnosis
WILL require 2nd surgery
can seed cancer cells
4
Q
intracapsular
A
rare
debulking- cut into tumor- leaving gross disease behind
5
Q
marhinal/cytoreductive excisions
A
lipomas and benign masses
maligant lesion- goal is microscopic disease
6
Q
wide excision
A
- 2-3 cm border
- 1 fascial plane deep
7
Q
radial excision
A
remove entire compartment
- amputation
- hemipelvectomy
8
Q
what do you want on histopath
A
- tumor vs other
- benign vs maligant
- histologic type
- grade
- margins
- lymphovascular invasion
