Which tests may be used in a bedside cognitive assessment?
MMSE
MOCA
RUDAS
ACE 3
What are the differences between the bedside cognitive assessment tests?
MMSE has a strong focus on memory
MOCA assesses frontal lobe function
Both require the patient to have a certain level of education so this is overcome with RUDAS
What questions are important to ask when taking a history for dementia?
What daily activities have been affected?
Rapid forgetting
Word finding difficulties
Safety issues: falls, getting lost
Withdrawal from social activity and apathy
Weight loss
Personality changes
What investigations should be done into dementia?
MRI of brain (rule out, medial temporal lobe atrophy in alzheimers)
B12/folate, TFTs and routine bloods
ECG (baseline)
PET scan shows how much glucose used by different parts of the brain
LP (amyloid:tau)
What is the pharmacological treatment of dementia?
Remove meds that impair cognition: anti-cholinergics and opiates
Cholinesterase inhibitors stop ACh breakdown
NMDA receptor antagonist
Avoid alcohol, control BP and comorbidities
Name 3 cholinesterase inhibitors
Donepezil
Rivastigmine
Galantamine
Name an NMDA receptor antagonist
Memantine
What factors affect the prognosis of a patient with alzheimers?
Weight loss
Functional loss
