Where can you get cancer in the digestive system?
Pretty much everywhere
Define cancer
A disease caused by an uncontrolled division of abnormal cells in a part of the body
Define primary cancer
Arising directly from the cells in an organ
Define secondary cancer
Spread from another organ, directly or by other means (blood or lymph)
Where can secondary tumours commonly form?
Liver, lung and brain
Where are squamous epithelial cells found?
Upper 2/3 of oesophagus, rectum
Where is glandular epithelium found?
Throughout the gut
What cancers do squamous cells form?
Squamous cell carcinomas
What cancers do glandular epithelium form?
Adenocarcinomas - most cancers of the GI tracts
List two neuroendocrine cells?
Enterochromaffin cells
Interstitial cells of Cajal
What cancers do enterochromaffin cells form?
Carcinoid tumours
What cancers do interstitial cells of Cajal form?
Gastrointestinal stromal tumours
What cancers does smooth muscle form?
Leiomyoma/Leiomyosarcomas
What cancers does adipose tissue form?
Lipomas
What is dysphagia?
Difficulty swallowing
How is oesophagus divided into?
Thirds: Cervical oesophagus, middle and lower oesophagus
As you go down the oesophagus how does the structure of the muscle change?
Increased smooth muscle less skeletal muscle
What are the two types of oesophageal cancer that can arise?
Adenocarcinoma
Squamous cell carcinoma
Where does adenocarcinomas arise?
Lower 1/3 of oesophagus
What is associated with adenocarcinoma?
Acid reflux
Where does squamous cell carcinomas arise?
Upper 2/3 of the oesophagus
What is associated with squamous cell carcinoma?
Acetaldehyde pathway - alcohol stresses squamous cells
Describe the progression from acid reflux to cancer
Oesophagitis (inflammation) -> Barrett’s (metaplasia) -> Dysplasia -> Carcinoma (neoplasia)
What does chronic exposure to acid in the oesophagus cause?
Injury, ongoing inflammation, cytokine drive eventually leading to cancer
