Mx of Tinea Corporis, Tinea Cruris?
Mild = topical antifungals (terbinafine or clotrimazole)
Systemic & Severe = oral terbafine (1st), oral itraconazole (2nd)
Tinea capitis mx?
Systemic antifungal = griseofulvin or terbinafine
2nd line - itraconazole or fluconazole
Can add topical antifungal shampoo in some
When are you likley to get IBS following gastroenteritis? transient lactose intolerance?
IBS = After Campylobacter jejuni infection
Transient lactose intolerance = After viral gastroenteritis
What is labial fusion? consequence? mx?
Labial fusion is a benign condition = labia minora are joined together
Common in girls <7y usually resolves by puberty - develops at 1-2y
topical oestrogen 4-6w if signs of significant symptoms (if trapped urine
which is causing terminal dribbling and vulval inflammation opt for surgery)
Acid base imbalance in CAH?
Metabolic acidosis, hypoglycaemia, hyperkalaemia, and hyponatraemia
Low aldosterone -> high K, low na (k excretion also takes back h+ causing acidosis)
Low cortisol causes hypoglycaemia
What is the triad of reactive arthritis? what does this commonly follow?
Conjunctivitis, urethritis, and arthritis
Extra-articular infection (GI - c.jejeuni or urogenital - chlamydia)
What is lochia?
Lochia is vaginal discharge consisting of blood, mucus, and uterine tissue that
occurs for up to 4–6 weeks following delivery
(can become heavier during breastfeeding due to NE reflex that stimulates uterine contractions)
What should be avoided in older pts / imbalance w/ urge incontinence? what should be given?
Antimuscarinics (eg. tolterodine, darifenacin, or oxybutynin)
Give MIRABEGRON instead (B3-agonist)
What is prolonged 2nd stage of labour?
more than 1 hour in multiparous
more than 2 hours in primiparous
When should ventous be preferred over forceps?
NB forceps cant rotate so if rotation is required generaly use ventouse
K forceps have some rotational ability
What is dissociative fuge?
A subtype of dissociative disorder characterised by amnesia and purposeful travel beyond the usual everyday range
what is trance and posession disorder?
Type of dissociative disorder which causes
temporary loss of personal identity with full awareness of the surroundings
What is oppositional defiant disorder compared to CD?
Oppositional defiant disorder is a milder type of CD that occurs in children
under 11 years old and is characterised by arguing with and disobeying adults
without the extreme behaviours present in CD
How to distinguish between psychotic mania and shcizoaffective?
Schizoaffective = both psychotic and affective
symptoms are present to the same extent
Mania / Depression = psychosis only at worst extents of affective disorders and mood-congruent
