What can you do when switching someone over from oral opioid to fentanyl patch?

A

Patch takes ~12 hours to work, put patch on with last dose of the oral medication. Assess for dosage change after first 3 days (one patch; 72 hours), then every 6 days after that

Can cause QT prolongation, so use of other meds that prolong QT like Citalopram, Cyclobenzaprine, and Quetiapine should be looked at closely
