10 22 2014 Seizures Flashcards
What would you see if seizure was localized to medial vs. lateral temporal lobe
medial: staring, posturing, automatisms, postureing, fear
Lateral: vertigo
Frontal Localization of seizure
- dorsolateral?
- SMA
- Orbitofrontal
brief, bizarre, nocturnal
- contralateral vision loss
- fencing, posture
- elaborate, sounds, smell
What kinds of seizures would you see coming from the parietal lobe?
Sensory – sights, smells, taste, sounds
What kinds of seizures would you see with seizures coming from occipital
form visual phenomena
EEG pattern:
3 Hz spike and wave
Absence Seizures
EEG pattern:
General spike and wave or polyspike and wave
Primary Generalized Seizures; Primary genralized epilepsy (polyspike wave)
EEG:
Slow spike and Wave
Lennox- Gastaut Syndrome
- 2-6yrs of age
- frequent seizures and different types of seizures
- developmental delays
EEG:
high amplitude and irregular waves
Background is chaotic and disorganized
- non-interpretable EEG
-Quiets down for a few seconds
Hypsarrhythia: abnormal interictal pattern
EEG:
Rapid spike wave (> 3.5hz); polyspike wave
Juvenile Myoclonic epilepsy
- ION CHANNELS (idiopathic) (aka genetic)
- 12-18 years of age
- involuntary twitching of muscle or group of muscles.
- Tonic- clonic (grand mal) seizure
EEG:
abnormalities in only on one side of the brain.
Waves look as if making an X.
Focal sharp and wave
EEG:
Spike on one lead is facing the spike o f another in a specific area of the brain.
Focal Spike and Wave
Type of simple partial seizures?
- motor
- sensory
- psychic
- autonomic
NO LOSS OF CONSCIOUSNESS
-may also give a warning sign : smell, aura
Types of complex partial seizures
Impaired consciousness
- usually medial temporal lobe.
- usually lasts 1-2 minutes
- looks like day-dreaming/ staring
- Automatisms: move their mouth, pick at the air or their clothing, or perform other purposeless actions
Types of primary seizures
Tonic Tonic-Clonic Myoclonic Absence (petit Mal) -lapses of awareness, sometimes staring Atonic -- drop seizure (fall to ground)
