- impaired concentration
- appetite
- psychomotor agitation/retardation
- sleep disturbance
total of 5 for 2 weeks or more → MDD!
bipolar affective disorder
elevated, expanded, or irritable mood
if yes, next step…
DIG FAST
total of 4 or 5 over 7 days → MANIC EPISODE
hypomania
still using the dig fast criteria (total of 4 or 5)
BUT
less than 7 days!!!
minor depression
similar criteria to major depression: SIG E CAPS
BUT, less than 5 criteria
for 2 weeks duration
minor depression + hypomania?
cyclothymia!
psychiatric evaluation
equivalent of H&P when chief complaint is psychiatric in nature
if CC NOT psych-related:
include psych history with PMH
if CC is psych-related:
create new heading “Past Psych History”
MMSE
screening tool for dementia.
NOT the Mental Status Exam - should not be confused!
things you should list in HPI for psych
- onset: timing of problems
- duration: how long (constant/intermittent)
- assoc signs/symptoms (pertinent +/-)
- modifying/relieving factors
psych ROS questions
- depressed mood and/or anhedonia
- elevated, expansive, irritable mood
- SI/HI (suicidal/homicidal ideation)
- are you a nervous person?
- delusions, hallucinations (+ sx of psychosis)
- CAGE; illicit substance use
