Patients condition is serious enough and is deteriorating enough to be life threatening - serious pain or time crucial
- airway risk - stride or drooling
- circulation compromise
- BSL less than 3mmol/L
- drowsy - GCS less than 13

A
Patients condition may progress to life threatening - may lead to significant morbidity or humane practice mandates relief of severe discomfort or distress
- severe hypertension
- moderate/severe blood loss
- SaO2 90 - 95
- Seizure
- Head injury
