what is allergic conjunctivitis presentation? affects vision?
- bilateral red, itchy, watery eyes
- have normal vision & either seasonal or identifiable trigger
what type of hypersensitivity is thyroid eye disease?
type II
what is presentation of thyroid eye disease? (mnemonic)
s = soft tissue involvement
p = proptosis (bulging out eyeball)
e = extraocular muscles
c = corneal involvement
s = sight loss
what is presentation of episcleritis? treatment?
- segmental redness with dilated episcleral vessel (mild red eye)
- watery eye
- no pain or vision changes
*generally just small infection with not much symptoms, self limiting and treat with lubricants & cold compress
what is episcleritis?
benign inflammation of episclera (thin layer of tissue between conjunctiva & sclera)
what is scleritis? what systemic diseases is it associated with?
inflammation of sclera - full thickness inflammation (more severe than episcleritis)
- assoc with rheumatoid, SLE, IBD, sarcoidosis, GPA
what is uveitis?
inflammation of uvea (middle layer that is iris, choroid & ciliary body)
- it’s a common varied and potentially blinding systemic infection
what is example of type III hypersensitivy?
peripheral ulcerative keratitis
what is an example of type IV hypersensitivity?
corneal graft rejection
what is presentation of scleritis?
- painful (wakes patient at night)
- pain on eye movements
- photophobia
- reduced visual acuity
- abnormal reaction to light
what is scleritis management?
oral NSAIDs (or oral steroids)
*immunosuppression for underlying disease
what is presentation of uveitis?
- dull/achy painful red eye
- reduced vision (maybe)
- photophobia
signs = red eye, hypopyon, synechiae
what is treatment of uveitis?
- topical steroids, mydriatics
*investigate systemic as mostly associated with autoimmune (reiter’s, UC, ankylosing, sarcoidosis)
