pain pallor pulselessness paresthesia perishing cold, paralysis
plantar flexion and inversion
lateral stabilizing ligaments (ant talofibular, calcaneofibular, posterior talofibular)
most common
anterior drawer test and inversion stress test
eversion and dorsiflexion
deltoid, anterior talofibular, bony mortise
less common
grading ankle sprains
Grade I - stretching and/or small tear of ligament
Grade II - incomplete tear and moderate functional impairment
Grade III - complete tear and loss of integrity of the ligament, severe swelling and ecchymosis, cannot bear weight. no definite stopping point.
crossed leg test
have pt cross their legs iwth the injured leg resting at midcalf on the knee to detect high ankle sprains
ottawa rules - ankle
pain in the malleolar zone AND either bony tenderness along distal 6 cm of posterior edge of either malleolus or inability to bear weight 4 steps immediately after injry and in the emergency department
ottawa rules - foot
pain in the midfoot region AND 1 of following
a) bony tenderness at either navicular bone or base of fifth metatarsal
b) inability to bear weight 4 steps immediately after injury and in ED
