01.07.2016 Intro Flashcards
4 main dimensions of clinical practice:
1) Knowledge
2) Clinical reasoning
3) Movement assessment
4) Consistent virtues
Red flags?
Fever, chills, night sweats Recent unexplained weight loss/gain Unexplained nausea/vomiting Paresthesia SOB Dizziness
Categories of pain & movement dysfunction:
1) Local neuro/MS
- Intra/Extra
2) Remote/referred neuro/MS
3) Systemic
Why patients move the way they do? (5 categories)
1) Pain
2) Neuromuscular
3) ROM
4) Habit
5) Structural deformity
Ottawa Rules
Lateral/Medial
1) Tenderness 6 cm post edge of fibular
2) Tenderness at tip of fibula
3) Inability to WB immediately after/ER
Midfoot
1) Tenderness at base of 5th MT
2) Tenderness at navicular
3) Inability to WB
Deformation curve: (2) regions
Elastic: stretch & tissue returns, can tolerate load
Plastic: tissue gets exceeded and micro tears, deformed
Stress/Strain curve: (3 grades)
1) Physiological range, before microscopic failure, toe region, linear region, straighten fibers
2) After microscopic failure to macroscopic failure
3) Macroscopic failure, rupture
Severity of injury (grades):
1) Minor tear
2) Incomplete tear
3) Disruption of ligament w/ instability
Effects of Immobilization (4)
1) Cartilage degeneration
2) Decreased mechanical & structural properties of ligaments
3) Decreased bone density
4) Weakness/atrophy of muscles
Assessment techniques (order)
- Gait analysis
- Functional mobility
- Observation
- Edema assessment
- Neuro screen
- ROM
- MMT
- Special tests/Balance
Edema mangement
- Manual therapy
- Modalities
ROM
- Joint accessory motion
- STM
- Motor re-education
Weakness
- Strengthen (power, Endurance, Control)
- Motor re-education
Intervention drive by (6)
- SINS
- Pain
- Altered ROM
- Loss of strength/endurance
- Altered balance/proprio
- Movement pattern not optimal
