What insect transmits Yellow Fever?
Aedes Aegyptii. The disease can also be transmitted by Blood transfusions and Breast Feeding.
What kind of virus is the Yellow Fever virus?
A Flavivirus. There ae 7 strains of this virus.
How does the African strain differ from the American strain?
An extra antigen.
How does one become immune to Yellow Fever?
Development of Antibodies
What are the clinical phases of Yellow Fever?
Phase 1- Fever with Jaundice
Phase 2- Period of Calm
Phase 3- Recurrence of Fever with Hepatogenic Bleeding with deteriorating LFTs, Myocarditis and CNS involvement.
Heavy proteinuria with above findings should make you think of Yellow Fever
How is the disease diagnosed?
Serum ELISA in first three days.Look for IgM and IgG abs.
What are the transmission cycles?
Sylvatic- monkey reservoir
Intermediate/savannah- humans and monkeys
Urban- Human reservoir
What is the incubation period of yellow fever?
2 to 8 days with a median of 4 days
What percent of those with Yellow Fever are asx?
85%. Only 2 to 3 % become hemorrhagic and jaundiced.
What test is done if you suspect a falsely positive for Yellow Fever?
A PRINT test (Plaque Reduction Neutralization test)
How long does protection from the vaccine last?
10 years. There are 2 strains, the 17DDfrom Brazil and the 17D-204 made outside of Brazil.
For who is the vaccine recommended?
Those greater than 9 months of age.
What are contraindications to using this vaccine?
Egg sensitivity, those less than 6 mos and those over 60 years, those with immunodeficiency or immune modulation for any reason.
