1 suspected drug- brand/name of D/ manuf for herbal / Batch no/ route of adm/ dosage/ date of adm/ indication
2 suspected reaction- description of reaction/ any trt given/ date of rxn start n stop/ whether it was SRIOUS
3 patient details- no patient name but identification numbers are helpful in case of referral. Sex/ age/wt if known. CONSENT
4 reported details- Name address of reporter (can be reached for further info)
