How does naltrexone work?
Competitive antagonist for opioid receptor
Blocks enjoyment from alcohol
How does Acamprosate work?
Gaba agonist
Anticraving
How does disulfiram work?
Deterrent
Acetyl aldehyde dehydrogenase inhibitor
Leads to build up of acetyl aldehyde which leads to hypertension, facial flushing, vomiting
Best given if supervised
What things do you want to ask in a drug misuse history?
How much they use, money can be a good indication
How many times a day - gives idea of severity of use
How they take it
Where they inject (tend to start with arms –> legs –> groin –> neck (v. dangerous))
What are the SEs of injecting drugs?
BBVs (HIV, HepB, HepC) Local infection/abscess Infective endocarditis Sepsis DVT/PE
What drugs can be given to help people get off of heroin?
Methadone
Butamorphine
