Approved form for the requisitioning of Schedule 2 and 3 CDs in the community
- Mandatory in community
- This applies to both requisitions for human and for veterinary use.
- Hospices (unless its not for a ward with the same legal entity) and prisons are exempt from the requirement to use the approved form.
Legal requirements for CD req are:
1 Signature of the recipient
2 Name of the recipient
3 Address of the recipient
4 Profession or occupation
5 Total quantity of drug
6 Purpose of the requisition

What req forms are used in prison?
- Hospital-style req forms (instead of a standardised form) are usually used
- Printed in a bound, book format
- Sequentially numbered with a carbon copy of each requisition to provide a robust audit trail.

CD practice issues
- Fax or photocopied requisition is not acceptable
- Req in writing must be obtained before supply of schedule 2/3 to most recipients
- In an emergency, a dentist can be supplied 2/3. Req must be supplied within the next 24 hours
- Messengers must have written authorisation that empowers them to receive the drugs on behalf of the recipient
Schedule 1 CD req
- A licence would be required for any healthcare professional to possess Schedule 1 CDs
- Pharmacists are reminded that they are not able to requisition Schedule 1 CDs
Processing requisition forms
When a requisition for a Schedule 1, 2 or 3 CD is received, it is a legal requirement to:
- Mark the requisition indelibly with the supplier’s name + address (i.e. the name of the pharmacy);
- where a pharmacy stamp is used this must be clear and legible
- Send the original requisition to the relevant NHS agency.
How long should pharmacies retain a copy of the req from the date of supply?
2 yrs from the date of supply.
When do processing requirements not apply?
When the supply is made by:
- Person responsible for the dispensing and supply of medicines at a hospital, care home, hospice, prison
- organisation providing ambulance services who must mark and retain the original requisition for two years
- By pharmaceutical manufacturers or wholesalers
- Against veterinary requisitions (the original requisition should be retained for five years).
Midwife supply orders
A registered midwife may use a midwife supply order to obtain the following CDs:
Diamorphine
Morphine
Pethidine.
What must the midwife supply order contain?
- Name of the midwife
- Occupation of the midwife
- Name of the person to whom the CD is to be administered or supplied
- Purpose for which the CD is required
- Total quantity of the drug to be obtained
- Signature of an appropriate medical officer
