Wounds Flashcards
University of texas-San Antonio ulcer classification
A: pre-/post- ulcerative lesion (grade 0) — superifical wound (grade 1) – wound penetrates to tendon or capsule (grade 2)– wound penetrates into joint or bone (grade 3)
B: + infection
C: + ischemia
D: + infection and ischemia
Wagener Ulcer classification
grade 0: no open skin lesions present (pre/post ulcerative)
grade 1: partial-thickness with no deep structure involvement (bone, joint, ligament, muscle)
2: full-thickness with deep structure involvement
3: 2 + includes abscesses, osteomyelitis, erythema, & edema
4: 2+ includes wet/dry gangrene isolated to toes & forefoot
5: 2 + includes wet/dry gangrene to entire foot
collagen usage
chronic non-healing wounds
- decreases protease activity and inflammation, increase cellular proliferation
hydrocolloid usage
dry to slightly moist wounds
- maintains moisture, insulates, provides autolytic debridement
hydrogel usage
dry to slightly moist wounds
- regulates fluid exchange, rehydrates wound bed
foam usage
wounds with moderate to heavy exudate
- absorbs exudate and provides cushioning
hydrofiber usage
wounds with moderate to heavy exudate
- can be used with compression or deep wounds, absorbs drainage
alginates usage
wounds with heavy exudate
- maintains moisture, absorbs exudate, hemostatic
anti-microbials usage
prevents bacterial growth
promogran prism matrix, cellerate RX, stimulen, triple Helix
