Wound bed prep Flashcards
Acute wound
progress sequentially through the natural phases of healing in a timely manner
High mitotic activity
Therapeutic levels of inflammatory cytokines
Low levels of proteases
Typically heal less than 21 days
chronic wounds
do not progress sequentially through healing
Frequently stuck in an inflammatory phase
low mitotic activity
increase levels of inflammatory cytokines
High levels of proteases
May persist months to years
Venous ulcers
Compress
Pressure injuries
offload
Reduce pressure and shear
neuropathic ulcers
Offload
Reduce pressure and shear
arterial ulcers
Protect and re-perfuse
Treatment for all chronic wounds
optimize nutrition
Moist wound healing
Pain reduction
criteria for adequate perfusion
Palpable pulse indicates at least 80 mmHg pressure within the foot
ABI- >0.6 and < 1.2
Audible triphasic, or biphasic sound with Doppler
if ABI is greater than 1.2 perform toe pressure indices > 30-55 mmHg
Venous filling time assessment 5 to 15 seconds
rubor of dependency - pallor is greater than 25 seconds and refilling in less than 30 seconds
