What are the GOS?
- in order to provide- you need to have a contract with the NHS england: individual and buisness
- As an indicual you have a othalmic perfomers list
How do you apply for the NHS site test?
GOS 1- blue form
Who is eligible for a GOS 1?
-under 16
-under 19 in full time ed
-0ver 60
- If you wear complex lenses- has a power of 10.00D or more in any one meridian
-Registered sight impaired or severely sight impaired
-prisoner or leave from prison
-patients suffering from diabetes of glaucoma - includes
patients with previously treated glaucoma or with high IOP at risk of glaucoma
What is the second part to the GOS 1 form?
-Date of your last site test (NHS or private)

What other patients are eligible for a NHS sight test?
- 40 or over with parent, bro,sis or child of a person who has been diagnosed as suffering from glaucoma
-individuals who are in receipt of:
income support
pension credit guarantee credit
income based job seekers allowance
income based employment and support allowance (not contribution based)
entitled to,or named on a valid NHS tax credit exemption certificate
are awarded universal credit - a member of the same family of a person who is in receipt of the above financial support
-patients named on a HC2 or HC3 form
What does the optometrist have to do after they have completed the sight test?
- They need to tick whether the patient has changed their prescription, or if its stable or they are eligible for a voucher
- They need to put their OPL number (national performer list number)- completed eye test to the general ophthalmic services and write their findings
What needs to be issued after the eye test?
an optical prescription (GOS 2)- green form
What does the GOS 2 need to tell?
Whether
- a new/changed prescription was issued
- an unchanged prescription was issued
- no prescription was required
- the patient was referred to their GP or opthalmic hospital
- A voucher was issued
What happens if the patient gets help for the cost of their glasses >
They fill out a GOS 3 form- yellow
What is a GOS 3 form?
Optical voucher and patients statement
What is written in the GOS 3 form?
- Have patients prescription and type of voucher
- patients signs part 1 to confirm eligibility and part 2 after receipt of spectacles.
What voucher does a patient receive with a single vision pair of glasses?
A-D
What voucher does a patient recieve with a multifocal or bifocal pair of glasses?
E-G
What voucher does a patient recieve if they get 2 pairs of single vision (distance and near) ?
A+A- allowing them to get 2 pairs of glasses
What does the optometrist need to do?
- Sign it

What can the GOS 3 voucher help with?
-cost towards spectacles or contact lenses
-optom can issues a vouchers to those eligible if :
there is a change in prescription
spectacles need replacing due to fair “wear and tear”
What is the lifetime of a voucher?
- 2 years from date issue - onlt if the patient is still eleigible to help with costs (e.g age)
- if patient looses voucher , you cant issue another voucher they need permission from local authorities
What is a GOS 4?
- NHS optical repair/replacememnt voucher- pink form- definitely eligible for kids
Who is eligible for GOS 4?
- UNDER 16
- 16-18 in full time ed
- benefits (etc, etc)
- complex lenses
- prisoners on leave from prison
Who can the GOS 4 help for?
- automatically available to under 16s
- older patients may be eligible however :
- not automatic
- only applicable if damage a result of illness
- nobody else is eligible
What is a GOS 5 form?
Claiming for a private sight test- orange form
- completed by patients who have a HC3 certificate
- claiming for part help towards the cost of an eye examination
What is a HC3 form?
HC3 gives you part help towards the cost of your eye test
What is a GOS 6 form?
Application for a domiciliary eye examination
