causes, clinical features and management of respiratory conditions
asthma
chronic inflammation and reversible bronchoconstriction that occurs when allergens, irritants and infections enter airways
atopic disease
symptoms:
wheeze,
breathlessness,
cough
tight chest
prolonged expiration
chronic asthma treatment
SABA - reliever for everyone as required
LABA/ICS - preventor
acute asthma management- emergency
- bronxhodilator therapy immediately/oxygen
- corticosteroids within first hour
assessing chronic asthma severity
mild- day time symptoms more than once per week but not every day OR night symptoms more than twice per month but not every week
mod- day symptoms daily or night symptoms more than once per week or sleep/activity restrcted
severe- continuous day time symptoms or night time symptoms or night/activity restrictions
when to refer
emergency
new shortness of breath at rest
altered level of consciousness
increased work of breathing
chest pain with inspiration
signs of cyanosis
GP
recurrent or chronic symptoms that pt didnt previously have
chronic obstructive pulmonary disease
chronic, progressive and incompletely reversible airflow limitation due to
1. inflammation in small airways
2. alveolar destruction (emphysema) -reduced gas exchange
clinical features of COPD
smoker - 20/day for 20 years history
persistent, productive cough
progressive breathlessness, worse on exertion and during infections
COPD treatment/management
-smoking cessation
-flu vaccinations
-long term oxygen therapy
-pulmonary rehab
combination of SABA, ICS, and antibiotic
pneumonia
acute inflammation of lungs due to infection
symptoms
fever
cough
sputum
chest pain
breathless ness
acute bronchitis
acute infecction of large airways -typically viral
