What is the well’s score used for?
Used in outpatients and A&E, low risk and a negative D-dimer can rule out a DVT, meaning an ultrasound isn’t needed.
How are the DVT symptoms scored?
Calf swelling more than 3cm than the other leg +1
Collateral non varicose superficial veins present +1
Entire swollen leg +1
Localised tenderness along the deep veinous system +1
Pitting oedema confined to symptomatic leg +1
Paralysis, paresis, recent plaster immobilisation of the lower extremity +1
What are the conditions for a cancer history to be scored?
Treatment or palliation within 6 months +1
How long must someone be bedridden for it to be a risk factor?
3 days
What score does a previous DVT give?
+1
What reduces the score?
An alternative diagnosis of DVT is likely -2
How is a score interpreted?
Less than 2 points moderate risk
Above 2 high risk
How do you treat a DVT?
Apixaban/ rivaroxaban first line, if not suitable LMWH for 5 days
